Application for Charitable Contribution

Thank you for your interest in CommonWealth Central Credit Union's Charitable Contribution award. This special award is presented to non-profit organizations located in Santa Clara County and demonstrating a financial need to continue their services or special program. If your organization is selected as one of our recipients, you will be contacted by phone. Money is awarded at our Annual Shareholders meeting in April.

Please complete all sections of the application, and remember to include the documents requested with your application. You may mail the completed application to the address below or drop your application off at any one of our branches.
